We appreciate the confidence you place in us when you refer your patients. Our goal is to achieve a seamless transition, so patients feel like our office is an extension of yours.
Keep in mind that the more information we have about your referral, the better the service we can provide. We want to reinforce your recommendations, so we can be consistent in your treatment planning. Please include answers to the following questions:

What are the tooth numbers and/or quadrants relevant to the procedure?

What procedures or treatments have you performed on the patient?

What you are planning to perform in the future?

• |
Do you have x-rays taken within the last year that you can mail or email (in JPEG format)? We recommend an FMX for comprehensive cases, a PA for specific cases (excluding soft tissue grafting), and a PANO for posterior implant cases. Email is preferred, because we can import the scans directly into the patient's chart.

Have you filled out our online Patient Referral Form? You may also download the form and fill it out manually.

Has your patient been informed that we will need to perform an exam before we can schedule surgery? Note that the exam will allow them to better prepare for their procedure, in terms of planning, pre- and post-op care, and finances.

Have you scheduled your patient for their appointment with us? We've found that referral follow-through is most successful when patients are scheduled while they are at your office. If you call while your patient is waiting, we promise to make scheduling them a priority.

Have you encouraged your patient to continue care at your office while seeing us, even if it's only for a restorative exam? This keeps them up to date on their overall treatment plan. |
Are you forwarding us copies of x-rays for patients on alternating recall between our offices? Doing so avoids the need to inconvenience your patients by taking additional films. For BWX, vertical are most helpful to us so we can evaluate properly from a periodontal standpoint.
